The AI never assumes gender from caller-ID name or voice. It captures chosen name + pronouns from the caller themselves and uses them on every callback / text confirmation. Trans + non-binary patients are not misgendered on call one. Same protection for cis patients whose legal name differs from preferred name.

Care-type non-confirmation under HIPAA + state shield laws

Hostile callers asking "do you provide gender-affirming care?" / "do you do abortions here?" / "is my daughter being seen for hormone therapy?" get a hard-coded refusal: "I can't share details about the care our patients receive." Enforced at the system-prompt level — not optional, not bypassable.

State-AG + parental-coercion subpoena pushback

If we're served a state-AG fishing subpoena for gender-affirming or reproductive-care records, we notify you within 24 hours (unless gagged), assert HIPAA + state shield-law protections (CA, CO, NY, IL, WA, MN, MA, NJ, NM, OR, VT, MD, CT, RI as of 2026), and refuse to produce care-type data without a court order specifically addressing the shield-law exception. Crisis routing — 988 + Trans Lifeline + Trevor + Crisis Text Line

When a caller says they're in crisis, the AI shares 988 + the population-specific resource (Trans Lifeline 1-877-565-8860, The Trevor Project 1-866-488-7386, Veterans Crisis Line 988 press 1, Crisis Text Line text HOME to 741741) — then pages you immediately, regardless of after-hours rules.

    The AI says "pregnant person" not "mother" until told otherwise. Insurance card with deadname? The AI captures the legal name on the card silently + uses chosen name on the call. Medical-record naming honors clinician policy.

I run a trans-affirming OB/GYN in a hostile state — is the AI safe to use?

Yes, with the safeguards above. The AI never confirms identity OR care type. State-AG subpoenas get the 24-hour notification + shield-law pushback per /privacy → Legal process. Texas + Florida + similar hostile states: we've explicitly added shield-law alignment for CA / CO / NY / IL / WA / MN / MA / NJ / NM / OR / VT / MD / CT / RI shield-law states — those laws protect out-of-state patients receiving care via telehealth from your practice. Email us if your specific state regulatory posture needs review.

Will the AI misgender my patient if their voice doesn't match their pronouns?

No. The AI is instructed (safety.ts ALWAYS rule 10a) to never assume gender from voice, caller-ID name, or any other signal, and to use "they/them" until the caller offers pronouns themselves. When a caller offers a different name than caller-ID, the chosen name is used on every subsequent callback / text confirmation.

Stripe deplatforming risk for trans-care + reproductive-health practices?

Stripe historically has deplatformed certain trans-care and reproductive-health businesses without warning, citing "high-risk merchant category" reviews. RingDispatch billing is via Stripe; we have no special protections against this. If you're deplatformed by Stripe mid-month, your AI keeps working through the end of your paid period. We are exploring alternative payment processors for at-risk practices (Square + bank-direct ACH) — email if this is a current concern.

What about pediatric-trans-care intake involving minors?

Particularly sensitive. The AI never confirms a minor patient's identity OR care type to ANY caller, including a self-identified parent or guardian. "Is my daughter being seen here?" gets the standard non-confirmation. Parents seeking access to their minor's records must go through the clinician's documented release-of-information process — not the phone-AI. This protects you against parental-coercion lawsuits and against state-AG fishing for minor gender-affirming care records.

Crisis routing for OB-specific emergencies?

Yes. Ectopic pregnancy pain, severe 1st-trimester bleeding, postpartum hemorrhage, postpartum depression with self-harm risk — all trigger 911 / 988 / OB-on-call paging per safety.ts rules 11-12. Postpartum depression specifically gets 988 + a clinician page (it's a high-suicide-risk perinatal-period mood disorder).
